چکیده

Although there is increasing interest in the local delivery of drugs to the inner ear by applying them to the round window (RW) membrane, most drug application protocols have been empiricallybased. As a result, consequences of changes in delivery method, applied drug concentration or even small alterations in treatment protocols have been difficult to predict. Since direct measurements of drug concentration time courses (as required in phase I clinical studies) are not presently possible in the human inner ear, computer simulations provide a valuable tool for estimating drug concentrations in the inner ear for phase II clinical studies. The ultimate goal is to simulate drug movements in the inner ear sized appropriately to the human after topical application to the RW membrane.

Analysis of gentamicin kinetics in fluids of the inner ear with round window administration.

HYPOTHESIS That a theoretical basis for quantifying drug distribution in the inner ear with local applications can be established. BACKGROUND As methods of local drug delivery to the inner ear gain wider clinical acceptance it becomes important to establish how drugs are distributed in the ear as a function of time and for different delivery methods. METHODS The time course of gentamicin co...

Enhanced local bioavailability of single or compound drugs delivery to the inner ear through application of PLGA nanoparticles via round window administration

In this paper, the potential of poly(D,L-lactide-co-glycolide acid) (PLGA) nanoparticles (NPs) for carrying single or compound drugs traversing the round window membrane (RWM) was examined after the round window (RW) administration of different NPs to guinea pigs. First, coumarin-6 was incorporated into PLGA NPs as a fluorescent probe to investigate its ability to cross the RWM. Then, PLGA NPs ...

Cochlear pharmacokinetics with local inner ear drug delivery using a three-dimensional finite-element computer model.

HYPOTHESIS Cochlear fluid pharmacokinetics can be better represented by three-dimensional (3D) finite-element simulations of drug dispersal. BACKGROUND Local drug deliveries to the round window membrane are increasingly being used to treat inner ear disorders. Crucial to the development of safe therapies is knowledge of drug distribution in the inner ear with different delivery methods. Compu...

Quantitative interpretation of corticosteroid pharmacokinetics in inner fluids using computer simulations.

The delivery of drugs to the inner ear by applying them directly onto the round window membrane is a promising way to treat human inner ear disorders. To further develop this strategy, and to design controlled clinical trials, additional preclinical studies are necessary. It is especially important to derive the time course and total dose for the various target regions within the inner ear. Sin...

Round Window Membrane and Delivery of Biologically Active Agents into the Cochlea

Establishing efficient methods for local administration of drugs to the inner ear has great clinical relevance for the management of inner ear disorders. However, the administration route remains a critical issue. The most feasible approach for a non invasive drug delivery to the inner ear is application of medication to the middle ear cavity on the promise that it will diffuse through the thin...
